Innovative Carer Project reaches ‘1000’ milestone
The Teaming Up for Transition Project, (TUFT) part of Derbyshire Carers Association Carer identification project, has entered its second successful year working with GPs, health professionals and Carers. To date, the project team have identified and supported over 1000 new Carers in Derbyshire since our launch in 2012.
The Department of Health funded Teaming up for Transition initiative is the only one of its kind in the country and its success will surely lead to other areas duplicating work found here in Derbyshire.
The team works closely with partners in Derbyshire to ensure Carers are identified and referred for suitable support. The team also offer information at stands in royal and community hospitals and within GP surgeries.
Carers Week: The Derbyshire Carers Association Transition Team are holding Carer events to coincide with Carers’ Week (9-15 June). All events will be posted online at www.derbyshirecarers.co.uk

During our first year we have:
- Worked with 80 ‘Carers’ Champions’ based within GP surgeries throughout the county ensuring they are “carer aware”
- Delivered presentations to health professionals to raise the profile of Carers
- Delivered presentations to Carers to offer support and guidance
- Worked closely with GPs and health professionals to improve their understanding of Carers’ needs
- Attended Carer and Dementia Clinics to support carers
- Developed referral systems to create a strategic link with health centres
- Delivered Carer awareness training
- Attended “Flu Clinics” to identify Carers
